SMS Consent

Alema Health provides text message communications on behalf of independent primary care practices to support care coordination, appointment scheduling, and clinical follow-up. This page describes the SMS program and provides a method for patients to affirmatively opt in to receive these messages.

About This SMS Program

Alema Health operates as a HIPAA Business Associate of independent primary care practices ("Practices"). On behalf of each Practice, we send brief text messages to that Practice’s patients with care coordination prompts and links to a secure, practice-branded booking page where the full appointment context and scheduling functionality is presented.

Messages are sent only to patients of a Practice who have an established treatment relationship with that Practice. Patient phone numbers are sourced from each Practice's electronic health record where the patient has affirmatively opted in to receive SMS communications.

What Messages You'll Receive

Messages from this SMS program are brief and contain only the minimum necessary information. Examples include:

Reminders about recommended preventive visits, such as your Annual Wellness Visit
Follow-up after a recent hospital discharge or emergency department visit
Appointment reminders and rescheduling prompts
Care plan check-ins for chronic condition management

Each message identifies your Practice by name, includes a brief action prompt, and includes a secure link where the full clinical context is available within an authenticated session. Message frequency varies based on your care needs and your Practice's outreach cadence, typically 1–4 messages per month.

How to Opt In

You can opt in to receive SMS communications from your primary care practice through Alema Health in any of the following ways:

By submitting the consent form below with your phone number, name, practice name, and checkbox confirmation. This is the publicly available consent method.
By replying START to a message from Alema Health
By completing your Practice's patient intake form and providing affirmative SMS consent during patient registration

Upon initial opt-in, you will receive a single confirmation message describing the SMS program. You will then receive care-related messages from your Practice through Alema Health as described above.

How to Opt Out

You can opt out of SMS communications at any time by any of the following methods:

Reply STOP to any message you receive. You will receive one final confirmation message acknowledging your opt-out, and no further messages will be sent.
Contact your Practice directly and request to be removed from SMS communications.
Update your communication preferences by replying to a text or contacting your Practice directly.

Opt-out keywords also recognized: STOP, STOPALL, UNSUBSCRIBE, CANCEL, END, QUIT, REVOKE, OPTOUT.

Privacy and HIPAA

Alema Health is a HIPAA Business Associate of each Practice and handles protected health information (PHI) strictly in accordance with our Business Associate Agreements and applicable law. SMS messages contain only the minimum necessary information and do not include diagnoses, conditions, procedures, or other detailed clinical content.
